SIMKES Visited Wonosobo Health Office
Related to study observation, SIMKES’s student visited to Wonosobo Health Office on Saturday, April 12, 2008 to observe the implementation of local government clinics’ information system (called SIMPUS). In this observation, the students were visiting Mojotengah government clinic and keeping a close watch on SIMPUS process since the patients’ registration, data entry, until they became a database which connected to all Wonosobo government clinics.
The connected SIMPUS to Wonosobo Health Office is enabling the staff of health office controlled the activities form each local government clinic, in such the way that the planning and program health management for local government clinics is better and more accurate.
The SIMPUS have been implemented in Wonosobo since 2006 and at the first time it was a spontaneous and never been planned before by the management. With the fund of Rp 2.5 billion and aiding from a son of head of Health Office (has replaced at the moment), network for the SIMPUS online was prepared. In the early stages, most of the people in Wonosobo Health Office did not know what they should do with the SIMPUS outputs for supporting the organization planning. At first, it was only the “winding machine” which was working and no other actions which have done. To cover those problems, Wonosobo health office held SIMPUS training for the employees in order they could operate and utilize the SIMPUS optimally. A good will to improve the regional health program makes Wonosobo Health Office always develop the SIMPUS utilization all the time.
In 2007, computers of SIMPUS were completed with web camera for teleconference and SIMPUS operators have been ready with computer and information skills. Finally, in this year, internet has been installed at all regions. However, several regions (i.e. Wadaslintang) were experiencing internet disruption because of the topography condition.
From the Wonosobo experience in implementing SIMPUS, we can get a lesson that the SIMPUS implementation can run well if it is supported by people who have strong commitment to bring it into reality.

Under the supervisory of Prof. dr. Sugeng JM, DAP&E, M.Sc. dan Prof. dr. Prototype Development of Open Source-Based Personnel Management Information System (SIMKA) at Pariaman Health Office in the Year of 2007

Background: Pariaman Municipality’s Health Office is one of important institutions which support the quality of health care services. Until now, it has some problems about manual and conventional staff and health officers records, staff promotion program, and unsophisticated and incomplete staff data. Actually, government has already provided the SIMKA software for free, but it has not been implemented yet. The causes are the system is not open source so that it is difficult to update data, and there is no trained and skilled attendants to operate the system. Therefore, implementation of the open source-based SIMKA, perhaps, is helpful in monitoring human resources’ potentials.
Objective: The study was aimed to develop a system of staff management information at Pariaman Municipality’s Health Office in supporting the availability of managerial task implementation and improving the quality of decision making. It included needs assessment, prototype designing, prototype design implementation, and prototype design evaluation.
Methods: The study was a qualitative research with an action research method to explore the need of information deeply that would be analyzed, presented, and approved as design of staff’s information system.
Results: The result of the staff information system was the integrated and comprehensive data to produce a complete and comprehensive system. After the prototype implementation, there was positive responds from the management staff and related parties; they could access the data easier and faster only by inserting a code. The prototype affected the data processing which could be done efficiently, gathered unlimited data, quicker data browsing, and properly data saving.
Conclussion: Prototype by modern technology could improve the monitoring of staff activities, and the effectiveness of used application because the prototype was applicable. It is suggested that the staff sustain the system in order it can be used in a long time.

Model Design for the Development of Information System for Management Program at West Pasaman District Health Office

Background: West Pasaman District Health Office is an organization that responsible for managing the health data. A major problem that has been identified regarding the health data are the accuracy, completeness and timeliness of the data. It also has been identified that there are 57 reporting forms that create many duplication and inefficiency. To address this issue, the development of a new model based on the need from the users has been proposed.
Objectives: This study was aimed to identify input and output indicators from the users and develop a new model design for information system at West Pasaman District Health Office
Methods: This study applied a descriptive qualitative method. Data were collected trough in depth interview, group discussion and observation. The subjects were chief of all programs and sub programs at the district health office. The problems of an existing management information system has been identified and a model design for health management information system development was made. The model design was described by using data flow diagram (DFD), and entity relationship diagram (ERD).
Results: The accuracy, completeness and timeliness of the existing 57 report forms has been analyzed. The duplication of these data has been removed and the result were shown by using tables, graphics and maps. These data are used as a district report to the province. This study also showed that 20 reporting forms from the primary health centers can be integrated to other reporting forms. As a result, the model design was described by using DFD and ERD. This model later on can be used fro the development of computer-based information system at West Pasaman District Health Office.
Conclusion: A model of the information system for the management of health programs at the West Pasaman district has been developed aiming to minimized data duplication and increase accuracy, completeness and timeliness.

Evaluation of Implementation of Transaction Information System in Puskesmas in Bantaeng Regency South Celebes Province

Background: Health information system is one of four health development major strategies. Health Office of South Celebes Province has tried to implement an electronic information system in local government clinics (Puskesmas) which is named Puskesmas’ Transaction Information System (SITRAPUS) in Bantaeng District since 2006. However, its performance and effectiveness have not been evaluated yet.
Objectives: The research aimed to evaluate the implementation of SITRAPUS in Bantaeng District from aspects of development process, the output, the barriers and entries during the implementation.
Methods: This was a qualitative study. The data was gathered by in-depth interview and observation in local government clinics, Bantaeng District Health Office, and Health Office of South Celebes Province.
Results: From the development process, the result showed that operator was not too involved in the process of SITRAPUS designing, the use of SITRAPUS was not well socialized so that the users were not familiar with the system, there was no a division which managed the information system, less supervisory, and there was no technical support for the SITRAPUS maintenance. The SITRAPUS output were disease reports but they were not accurate and irrelevant with the organization needs. Nevertheless, the users felt that the system could make their works easier, especially in data searching. The SITRAPUS output were used by the local government clinics in calculating the retribution, meanwhile data access to the district was not up to date and timely.
Conclusions: The SITRAPUS implementation in Bantaeng District was not optimal, because this implementation did not accordance to the system development life cycle and the output was only report.

Evaluation of Computerized Registration and Out-Patients System Implementation At Mayjen. H. A. Thalib Hospital, Kerinci

Background: The 14th target of 2007-2009 National Information System development planning is utilization of the on-line system step by step. By 2009, perhaps, entire health elements in districts and provinces, including central hospitals and local hospitals, will implement the on-line system optimally. The initial computer-based hospital management information system implementation in Mayjen H. A. Thalib Hospital, Kerinci District was in registration unit and outpatient department. This supported the willingness to improve public services and to increase the number of inpatients as well as outpatients.
Objective: This study aimed to evaluate computer-based registration and output systems in Mayjen H. A. Thalib Hospital, Kerinci District based on Davis’ theory about technology acceptance model. Several aspects of the system were evaluated, such as user perceived benefits, users’ attitude toward the use of technology, ease of use, and users’ attitude and behavior toward computerized system.
Methods: This was a case study by using descriptive explorative study design to explore and classify a social phenomenon. Data were gathered by conducting in-depth interview to 11 respondents.
Results: The results showed that users agreed upon the implementation of HMIS because it was user friendly, and it, significantly, improved the users’ performance. Generally, users accepted and satisfied with HMIS. For the next development, they hoped that computerized system would be applied to entire units in the hospital.
Conclusion: Strong commitment from all staffs to build a better hospital management and also local government support delivered success in HMIS implementation. The users accepted HMIS because of its benefits and easiness so that it would improve the quality of public service.

Spatial Analysis of Malaria Cases During 2006 – 2007 in Lahewa Sub District, Nias District, Province of North Sumatera

Background : Nias District, Province of North Sumatera is one of malaria endemic areas which is influenced by the climate, heavy rainfall, mountainous topography, low socio economic status and education, most of the origin people work as farmers. Number of malaria cases in Lahewa Sub District were 1.432 clinical malaria cases (59,70%), 347 positive malaria cases (14,47%) in 2005, then they were 1.382 clinical malaria cases (44,13%) and 343 positive malaria cases (11,47%) in 2006. Therefore, it needed a geographical mapping for malaria cases in Lahewa Sub District.
Objective : This study aimed to map the spatial distribution of malaria cases and environment factors in Lahewa Sub District, Nias District in the year of 2007.
Method : This was a cross sectional study with analytical descriptive survey. Sample in this study were 84 patients with malaria symptoms, who lived in Lahewa, and had visited to the clinics during August 2007. Patients’ addresses were identified to determine coordinate points by using the Global Positioning System (GPS). The data were analyzed by using bivariate analysis with Chi Square test and spatial analysis with Sat Scan, Geode and Epi Info.
Results: This study resulted malaria cases associated with the use of mix garden (p = 0,0059) and humidity (p = 0,0309). However, malaria cases were not related to distance to health facilities (p = 0,084), the use of land as rice fields (p = 0,1405), the use of swamp area (p = 0,5442), the use of land as ponds (p = 0,6647), residential area (p = 0,0511), quantity of rainfall (p = 0,2379), and air temperatures ( p = 0,4513).
Conclusion: Malaria cases in Lahewa were not influenced by distance to health facilities and the use of land rice fields, ponds, residential area and swamp area, also the quantity of rainfall and air temperature. They were influenced significantly by the use of land as mix garden and humidity.
